Whether it is to help you with personal care, shopping, and housework or just someone to befriend, our home support service aims to support you to continue to live independently at home.
By providing the right sort of assistance in the way that people want it, the Home Support Service aims to support people to live fulfilled and satisfying lives in their own homes and communities where they belong, are known, and often have lived for a long time.
Because Dementia Care Partnership recognises what most people need and want, irrespective of any difficulties, is to be in their own homes and in control of their own lives as far as is possible, we make this the main focus of our services to people.
We provide a city-wide home support service in Newcastle. Our trained Home Support workers visit clients in their own homes from a minimum of 30 minutes to 24 hours, seven days a week offering assistance, support and encouragement in all aspects of personal care tasks. This helps maintain individuals’ personal skills and abilities and a sense of personal identity. We recognise that people need to remain as independent as possible and are keen to promote this. We also provide practical assistance with household tasks, shopping, etc.
How do I access the Home Support Service? Most of the referrals we receive are from social services who make referrals or purchase the service on your behalf following a comprehensive assessment of your needs and requirements.
However, you can contact us directly for our services. If this is the case one of our trained home support co-ordinators will visit you in your home to carry out an assessment of your needs and requirements and explain about our services.
A trained an experienced support worker would then be allocated to you.
Monitoring and Review All our services are monitored following 48 hours after setting up, thereafter 6 weeks, three months and so on, depending on the need and complexity.
